This case report presents the terminal phase of a patient with refractory neuropathic pain caused by invasion of breast cancer into the plexus brachialis. We want to remind that neuropathic cancer pain can be very difficult to control even with the use of many pharmacological and non-pharmacological approaches Refractory pain is an indisputable indication for palliative sedation; the correct timing and management of adequate depth of sedation can be challenging.
